Every published form has a public address that ends in a random code. With the Custom link, you replace that code with a short alias that is easy to say over the phone and to print on a poster. At ClÃnica Vitta, an aesthetics clinic in Miami, the front desk no longer texts an unreadable code: they say "f.wisedataforms.com/vitta-intake" and the patient types it right the first time.
The "Public link" card
Once the form is published, the Public link card shows up at the top of the form, above the tabs. It shows the address respondents should receive, with two buttons: Copy link and Open, which opens the published form in a new tab so you can check it.
If the form has an alias, the card already shows the link with the alias. A draft that was never published has no public link, and the card does not appear.
Creating the alias
In Forms › (form) › Share › "Custom link" card:
- Type in the Link alias field. The screen adjusts as you type: uppercase becomes lowercase, accents are dropped and spaces become hyphens. "Vitta Intake" becomes
vitta-intake. - Right below, It will look like: shows the full address exactly as it will be.
- Click Save link. The change applies right away, with no need to publish again.
Alias rules
- 3 to 60 characters: lowercase letters without accents, numbers and hyphens. No hyphen at the start or end.
- Some words are reserved, such as
admin, api, login, wwwandsupport. If you try one, the screen says "This link is reserved. Try another one." - It is unique across all WiseData Forms accounts, not just yours. Whoever saves it first keeps it. If it is already taken, you see "This link is already taken. Try another one." That is why including your business name helps:
intakeon its own is probably taken;vitta-intakeis not.
Changing the alias later
Changing the alias does not break the previous one. Every alias the form has ever had keeps opening the same form, and so does the original random code. That is what protects anything already printed.
In the example: Vitta printed posters with vitta-intake and, months later, started promoting vitta-form. The old posters still lead to the intake form, with no reprint.
Two consequences that often surprise people:
- A used alias stays tied to its form. It does not move to another form, not even within your account. Vitta cannot use
vitta-intakeon its post-treatment form, because that alias still opens the intake. Going back to an old alias of the same form is allowed. - Clearing the field and saving removes the alias from the link. The Public link card goes back to showing the random code, but the old aliases keep working.
Where the alias shows up, and where it does not
- Uses the alias: the Public link card and the embed code generated in the Share tab.
- Does not use the alias: QR codes and invite links point to the form's fixed address. So changing the alias never affects a printed QR code or an invite already sent.
If your account has an active custom domain, the link and the alias are shown on your domain (for example, forms.clinicavitta.com/vitta-intake).
Knowing where each response came from
You can add campaign parameters to the end of the link, such as ?utm_source=instagram, to tell apart people who came from your Instagram bio from those who came from the poster. How to build them and where to see the result is in the Response sources, UTM campaigns, devices and locations article.
Who can change it
Owner and Editor create and change the alias. A Viewer sees the Share tab but cannot change anything.
Where to go next
- Dynamic and static QR codes (Share collection) — the link on printed material, with scan counts.
- Embed the form on your website (Share collection) — the form inside your page.
- Invites: one link per person (Share collection) — when each person should respond only once.
- Custom domain (Share collection) — the link on your company's address.
- Response sources, UTM campaigns, devices and locations (Analytics collection) — campaign parameters in the link.
In short: the Public link card shows and copies the published form's address; in Share › Custom link you replace the code with an alias of 3 to 60 characters, unique across all accounts, and old aliases keep opening the same form forever, so nothing already printed breaks.
