Embedding puts the form inside your website, without sending people to another address. You choose how it appears, copy a ready-made code and paste it into the page's HTML. Views, the funnel and campaigns keep being counted as with any other visit. ClÃnica Vitta, in Miami, uses its intake form in two ways: inside the "Book your consultation" page and as a floating button on every page of the site.
Where to get the code
In Forms › (form) › Share › "Embed on your site" card. Pick the mode under How the form appears, adjust the options shown for it and click Copy code. The form must be published: you can copy the code earlier, but it only shows the form after publishing.
The four modes
On the page
The form sits inside the content, where you paste the code, and its height follows the questions: no second scroll bar shows up. This is the mode on Vitta's "Book your consultation" page. The code looks like this:
<div data-wisedata-form="vitta-intake" data-title="Intake form"></div><script src="https://f.wisedataforms.com/embed.js" async></script>
Popup
The code includes a button for your site; when clicked, the form opens centered over the page. It closes with the X, the Esc key or by clicking outside it. On phones, it fills the screen. The Button text option (up to 40 characters) sets what it says; left blank, it shows the form title. Since the button is a regular element of your page, your site can style it like its other buttons.
Slide-in
A panel shows up in the bottom-right corner after a few seconds, and the person keeps browsing with it open. It closes with the X. The timing comes from Shows after (seconds), from 0 to 120 (default 5). It shows every time the page loads.
Floating button
A button fixed to the corner of the screen as the person scrolls, which opens the form as a popup. You set the Button text and the Button position (Right corner or Left corner). Vitta uses "Fill in the form" on the right corner.
Where to paste it
- Paste the code into the page's HTML. In website builders, use the "custom HTML" or "code" block.
- In On the page mode, placement matters: the form shows exactly where the code is. In the other three, anywhere on the page works.
- Paste it once per page. For the floating button to show across the whole site, put the code in an area shared by all pages, such as the footer.
The page's campaigns carry over to the response
If the person reached your site through an address with campaign parameters, they are passed on to the embedded form. Example: Vitta's ad leads to clinicavitta.com/consultation?utm_source=instagram&utm_campaign=october. Whoever fills in the intake on that page shows up in Analytics as coming from instagram, campaign october — with no change to the code. How to read this is in the Response sources, UTM campaigns, devices and locations article.
After submission
- Thank-you page — shows inside the form's own frame.
- Redirect — if the form sends people to another address after submitting, the whole page navigates, not just the frame.
- Closing the window — in Edit › After submission, turn on Close the popup and slide-in after submitting. The window closes by itself 3 seconds after the thank-you page. It applies to the popup (including the one opened by the floating button) and the slide-in; On the page mode does not close, and a redirect takes precedence. Like the form's other options, it takes effect after you publish.
Alias and custom domain in the code
- The code uses the link alias, if there is one. Changing the alias later does not break code already pasted, because the old alias keeps opening the form.
- With an active custom domain, the code comes with your domain. If you remove the domain, code pasted with it stops working: copy it again and replace it on your site.
Where to go next
- Form link and custom alias (Share collection) — the address that goes in the code.
- Thank-you page and redirect (Appearance and after submission collection) — what shows after submitting.
- Response sources, UTM campaigns, devices and locations (Analytics collection) — campaigns arriving through the page.
- Custom domain (Share collection) — the code served from your company's address.
In short: in Share › Embed on your site you choose On the page, Popup, Slide-in or Floating button, copy the code and paste it once into the page's HTML; the published form shows there, inherits the campaigns from the page address, redirects the whole page when there is a redirect and can close itself 3 seconds after submission in the popup and the slide-in.
