Three fields for specific cases: the matrix rates several items at once, the video shows content without taking the person out of the form, and the hidden field stores where the response came from, with nobody typing it. All three are in the post-treatment survey of ClÃnica Vitta, a beauty clinic in Miami with two locations: Brickell and Coral Gables.
They are all in Forms › (form) › Edit › Fields card.
Matrix (several rows, same scale)
Use it when several questions share the same possible answers. Instead of four single choice fields with "Poor, Fair, Good, Excellent", a single matrix.
- Rows (what will be rated) — up to 20. At Vitta: Front desk, Punctuality, Room cleanliness, Esthetician's care.
- Columns (the scale, the same for every row) — up to 10. At Vitta: Poor, Fair, Good, Excellent.
- The matrix starts with two blank rows and two blank columns. It needs at least one of each and does not accept two rows (or two columns) with the same text.
Respondents pick one column per row. On a computer it shows as a grid; on a phone, each row becomes a list with the column name next to each option, with no sideways scrolling.
Answer required requires every row: the row left blank shows "Choose an option.". Without it, people answer only the rows they want.
In responses, each row becomes its own column in the exported spreadsheet ("How was your visit? — Front desk"). In the notification email, the answer reads "Front desk: Excellent; Punctuality: Good; …"; in the PDF, one matrix row per paragraph.
Video (YouTube or Vimeo)
A block just for watching, with no answer. It has a Title and a Video link. Vitta opens the survey with the video "Care in the first 48 hours".
Accepted links, always starting with https://:
- YouTube:
https://www.youtube.com/watch?v=…,https://youtu.be/…,https://www.youtube.com/shorts/…andhttps://www.youtube.com/embed/…. - Vimeo:
https://vimeo.com/followed only by the video number.
Playlist links, channel links, links from other sites or Vimeo links with anything after the number are rejected with "Enter a YouTube or Vimeo video link, starting with https://.".
Respondents see the player inside the form, with full screen, and the player does not take them away from it. The link you pasted is not shown in the form. In the response PDF, the video appears as its title and link, in text.
If the form is set to In steps, a step that has only a video or text blocks, with no question, is skipped.
Hidden field
A field respondents do not see. Its value comes from the form link. It tells you which campaign, location or partner each response came from without asking.
- Question — the name shown in responses and in the spreadsheet. At Vitta: "Location".
- URL parameter — the name that goes in the link. Only lowercase letters, numbers and _, up to 40. At Vitta:
location.
Then add the parameter to the form link. Vitta uses two links to the same form:
- Brickell:
…/vitta-survey?location=brickell - Coral Gables:
…/vitta-survey?location=coral-gables
Each response stores "brickell" or "coral-gables" in the Location field. For more than one hidden field, join them with &: ?location=brickell&source=whatsapp. If the link already has a ?, add yours with &.
Good to know:
- A link opened without the parameter stores the field empty. The value holds up to 500 characters.
- The hidden field has no Answer required setting and no condition to show, since nobody sees it. It can, however, be used as a condition to show other questions.
- Form embedded on a website: only parameters starting with
utm_pass from the website page to the form. To use a hidden field in an embedded form, name the parameterutm_source,utm_campaignor anotherutm_. - Someone who saves to continue later and comes back through the resume link does not bring the hidden field back: it is only read from the original link.
Where to go next
- Field types (Fields collection) — the other types and the common settings.
- Show fields based on the answer (Form logic collection) — using the matrix or the hidden field as a condition.
- Form link and custom alias and Embed the form on your website (Share collection) — where the link that takes the parameter lives.
- Response sources, UTM campaigns, devices and locations (Analytics collection) — what is already measured through
utm_parameters. - Exporting responses to CSV or XLSX (Responses collection) — the matrix and hidden field columns.
In short: the matrix rates up to 20 rows on the same scale of up to 10 columns, with one column per row in the spreadsheet; the video shows a YouTube or Vimeo link to watch inside the form; the hidden field stores the value of a link parameter, such as ?location=brickell, without showing to respondents.
