ID document, address, phone and amounts

ID documents, addresses, phone numbers and money are the fields where a wrong answer costs the most: an address with no city, a phone with no country code, an amount nobody knows the currency of. These fields check the format before submission and adapt to the respondent's country. The example is the "Treatment package sign-up" form at Clínica Vitta, a beauty clinic in Miami that also serves clients from abroad.

They are all in Forms › (form) › Edit › Fields card, in the Contact and documents group (and Numbers, for money).

ID document (any country)

For passports, national ID cards, driver's licenses or any other document.

  • Accepts 3 to 40 characters: unaccented letters, numbers, spaces, periods, hyphens and slashes. Otherwise the person sees "Enter a document number of 3 to 40 characters, using only letters, numbers, spaces, periods, hyphens or slashes.".
  • There is no mask and no check digit: every country has its own format. The answer is stored exactly as typed.
  • To tell whether it is the same person, case and separators are ignored: "ab-123" and "AB123" count as the same document.

Email and ID document can be the field that identifies the person in "one response per person".

Phone

The field comes with the country flag and code, and the person can switch country by searching its name. It opens in the country of the respondent's browser; if the browser does not tell a country, none is preselected.

  • The number needs 8 to 15 digits counting the country code. Otherwise: "Enter a valid phone number, with area code.".
  • The answer is stored with the country code in front (1 for the United States and Canada).

Address (with ZIP code)

A block with Country, Postal code, Street, Number, Apartment, suite, Neighborhood, State / province and City. The country opens like the phone's.

  • State and city come as searchable lists for the chosen country; when a list is not available, they become free text. Changing the country clears state and city.
  • The postal code accepts letters, numbers, spaces and hyphens, so it works for 33101 as well as for codes with letters.
  • A required address asks only for street and city, because many addresses have no separate number, neighborhood or postal code. Apartment is always optional.
  • An address filled in without a country is rejected with "Choose the country.".

Amount

In Currency you choose which currency the person types the amount in. The list has 20 currencies: US dollar, Canadian dollar, euro, pound sterling and Latin American currencies (Mexican, Colombian, Chilean, Argentine and Uruguayan pesos, Peruvian sol and others).

  • Starting currency: a new field starts in US dollars when the Form language is English or Spanish. Changing the language later does not change existing fields.
  • No cents: Chilean peso, Paraguayan guaraní and Colombian peso have no decimal places. The others have two.
  • On the respondent's screen: the symbol and separators follow the form language. The person types only digits and the amount takes shape ($1,250.00). Pasting "$1,250.75" also works: the thousands separator is not mistaken for a decimal point. Negative amounts are not accepted.
  • In responses: the amount shows with its symbol. In the exported spreadsheet it comes as a number, and a column right next to it ("Package budget — currency") holds the code, such as USD or EUR, so you can add up without mixing currencies.
  • Changed the currency after publishing? Each response stays in the currency of the version it was submitted in.

Clínica Vitta's form

  1. ID document (any country) "ID or passport number", required.
  2. Phone "WhatsApp", required.
  3. Address (with ZIP code) "Address", required.
  4. Amount "How much do you plan to invest in the package?", in US dollars.

A client from Miami keeps the country as is, fills in street, city, state and ZIP code. A client from Toronto switches the country to Canada and the state list becomes the Canadian provinces.

Where to go next

  • Field types (Fields collection) — the other types and the common settings.
  • Response limit, closing date and one response per person (Form logic collection) — using the ID document to block repeated responses.
  • Show fields based on the answer (Form logic collection) — asking for the address only when it matters.
  • Exporting responses to CSV or XLSX (Responses collection) — the amount and currency columns.

In short: ID document accepts any country's format; phone and address open in the right country and adapt their fields to it; Amount uses the currency you choose per field, with or without cents depending on the currency.
