The field type decides how people answer and what the form checks before letting them submit: an email without @ or a date like 02/31 does not go through. Picking the right type saves you from cleaning up messy answers later. As an example, ClÃnica Vitta, a beauty clinic in Miami, builds its "Book an assessment" form here.
Where to add and configure
In Forms › (form) › Edit › Fields card, choose the type in New field type (you can type to search) and click Add field. The bar shows at the top and at the end of the list. Each field opens as an accordion with:
- Question — what people read. Required, up to 500 characters.
- Type — can be changed later. When you change it, what the new type does not use is cleared (a choice field's options disappear if it becomes text, for example). From Single choice to Dropdown the options stay.
- Placeholder text — shows greyed out inside the empty box (up to 200 characters). Available in text, email, link, ID document, number, amount and dropdown fields.
- Help text — a line below the question (up to 500 characters). Use it for what prevents mistakes: "Use the email you check most".
- Answer required — without it, the person cannot submit. Required fields get an asterisk; the others, "(optional)". The field shows the Required badge in the editor.
The buttons next to each field move it up or down, duplicate and remove it; you can also drag it. A form has up to 50 fields.
Text
- Short text — name, job title, a one-line answer. Up to 500 characters.
- Long text — story, notes. Four-line box, up to 10,000 characters.
- Email — rejects an invalid address with "Enter a valid email address." and opens the email keyboard on phones.
- Link (URL) — website, profile, portfolio. If the person types
clinicavitta.com, the form addshttps://by itself; anything that does not look like an address is rejected.
Contact and documents
Phone, ID document (any country) and Address (with ZIP code) have their own country and format rules, explained in ID document, address, phone and amounts.
Numbers
- Number — quantity, age, weight. Accepts negatives and up to 4 decimal places; letters do not go in.
- Amount — with a symbol and a currency chosen per field; see ID document, address, phone and amounts.
Date and time
- Date — typed as MM/DD/YYYY in English forms, with the mask ready (forms in Portuguese or Spanish use day/month/year). A date that does not exist (02/31) is rejected with "Enter a valid date.".
- Time — HH:MM, from 00:00 to 23:59.
Choice
- Single choice — visible options, one answer. Good for up to about six options.
- Multiple choice — check as many as you like. When required, at least one is needed ("Choose at least one option.").
- Dropdown — the list stays closed and the person types to filter. It is the type for long lists, like the clinic's 30 treatments.
- Yes or no — two options side by side, no need to write them.
- Matrix (several rows, same scale) — see Matrix, video and hidden field.
Single choice, multiple choice and dropdown fields have up to 100 options; each one needs at least one and does not accept two with the same text ("This option already exists in this field.").
Rating
- Scale from 0 to 10 — eleven numbered buttons.
- Stars (1 to 5) — five clickable stars.
Terms, signature and hidden data
Terms acceptance and Signature have their own articles. The Hidden field does not show to respondents: it reads a value from the link and is covered in Matrix, video and hidden field.
Content
Blocks that only show something, with no answer, no "required" and no column in the responses:
- Text block — Title + Block text (required, up to 5,000 characters). For instructions: "Come without makeup on the day of your assessment."
- Section divider — a title (with optional help text) that splits the form into parts. With In steps chosen in the Appearance card, each divider starts a new step.
- Video (YouTube or Vimeo) — see Matrix, video and hidden field.
ClÃnica Vitta's form
- Section divider "Your details".
- Short text "Full name", required.
- Email "Your email", with the placeholder text name@example.com.
- Section divider "Your assessment".
- Dropdown "Treatment of interest", with 30 options.
- Date "Best day" and Time "Best time".
- Text block with the instructions for the day.
Anyone who types 02/30 in "Best day" sees the error right away, before submitting, and the front desk does not need to call to confirm the date.
Where to go next
- ID document, address, phone and amounts (Fields collection) — phone, ID document, address and currency.
- Matrix, video and hidden field (Fields collection).
- Show fields based on the answer (Form logic collection) — fields that appear only in certain cases.
- Terms acceptance and proof of consent and Electronic signature (Signature, terms and PDF collection).
- Steps, theme and logo (Appearance and after submission collection) — forms in steps.
In short: each field type checks the answer before submission; set the question, placeholder text, help text and required setting on each one, use a dropdown for long lists and content blocks to guide people without asking for an answer.
