Four choices define how the form looks to respondents: whether questions come all at once or in steps, whether there is a progress bar, the color theme and the logo at the top. The example is the intake form of ClÃnica Vitta, in Miami, in three parts: "Your details", "Health history" and "Consent form".
Single page or in steps
In Forms › (form) › Edit › "Appearance" card, How questions are shown has two options:
- Single page — all questions on one screen. Good for short forms.
- In steps — one step per section, with Next and Back buttons. Good when the list looks daunting: Vitta's intake form has 28 questions, and in three steps each screen feels short.
Steps are cut by the Section divider field, in the "Fields" card. Each divider starts a new step, and its title becomes the step title. At Vitta, there is a "Health history" divider before the health questions and a "Consent form" divider before the acceptance. Whatever comes before the first divider is the first step, without a title.
How it works for respondents:
- Next only checks the questions in the current step. A blank required field keeps the person there, with the notice on the field.
- There is one single submission, at the end. Nothing is saved as a response when moving between steps.
- If the submission flags an error in a field from another step, the form goes back to that step.
- A step with no visible question is skipped — both one emptied by conditions and one with only a Text block or Video. If you want a read-only screen, put at least one question in it, such as a Terms acceptance.
Progress bar
Show progress bar places the bar below the title. What it shows depends on the format:
- In steps — "Step 2 of 3".
- Single page — "60% answered", calculated on the visible required questions. With no required question, it counts all visible questions. A question hidden by a condition is left out.
Theme
In Theme:
- Light — the default.
- Dark — dark background for everyone.
- Automatic — follows the respondent's phone or computer: someone using dark mode on their device sees a dark form. Vitta uses this one.
When these choices apply
Format, progress bar and theme are part of the draft, like the questions: people opening the link only see the change after you Publish.
Logo
The logo shows at the top of the form, the thank-you page and the closed-form message. It is chosen somewhere else: Forms › (form) › Share › "Logo" card, under This form's logo:
- Account logo — the default. The same on every form using this option.
- Another logo — only for this form. When you pick it, upload the file right there. At Vitta, the "Vitta Men" line form uses that line's logo, while the rest keep the clinic logo.
- No logo — the form opens straight on the title.
The account logo is in Settings › Brand, which only the account Owner can access. Changing it there changes it on every form that uses the account logo. If the account has no logo yet, those forms open without one.
Accepted files: PNG, JPG or WEBP up to 512 KB; SVG is not accepted. The image is resized to at most 512 px. If you remove a form's own logo, it goes back to the account logo.
Unlike the theme, the logo applies right away, with no need to publish again.
Where to go next
- Show fields based on the answer (Form logic collection) — conditions that can empty a step.
- Kiosk mode (Appearance and after submission collection) — the form on a tablet at the front desk.
- Thank-you page and redirect (Appearance and after submission collection) — the screen shown after submitting.
- Draft, saving and publishing (Editing and publishing collection) — when changes reach respondents.
In short: in the Appearance card you choose Single page or In steps (cut by the Section divider), the progress bar and the Light, Dark or Automatic theme, all applying after you publish; the logo is chosen in Share › Logo — the account one (Settings › Brand), one just for the form, or none — and applies right away.
