Start from scratch or from a template

Every new form starts on the Templates page: blank or from a ready-made template with the questions already written. The New form button on the forms list takes you there. At Clínica Vitta, a beauty clinic in Miami, the client intake form came from the Facial intake form template and was adjusted in ten minutes.

Start from scratch

The first card on the page is Start from scratch. The Create blank form button asks only for the title and creates an empty form; you build the questions in the editor. Use it when what you need does not look like any template.

Choose a template

After the blank card come the 60 templates, in alphabetical order, each with a name, a description and the number of questions. The group buttons filter the list, and each one shows how many templates it has:

  • Health and beauty — intake forms (facial, body, hair, dental, lashes, micropigmentation and others), physical assessment, follow-up form and consent form.
  • Agency and business — contact, quote, lead qualification, briefings, client registration, order form, supplier registration.
  • Events, HR and education — event registration, RSVP, job application, employee onboarding, vacation request, enrollment.
  • Satisfaction — customer, service, product, restaurant, event, course, patient and workplace climate surveys.

The chosen group stays in the page address, so you can send the filtered link to someone on your team.

Preview before using

Preview opens a side panel with the template's questions in the order they will appear. Text, choice and terms questions show as fields; address, scale, stars, matrix and video show as a note describing what the person will find there. The preview shows what the template asks, not the final look of the published form. From there, Use template creates the form.

For the Facial intake form, the preview shows 23 items: identification (full name, date of birth, ID number, phone, email), health history (medical treatment, conditions, medication, allergies, pregnancy or breastfeeding, smoking), the skin questions (main concern, skin type, what bothers most, sunscreen, acids, isotretinoin, sun exposure, previous procedures) and the closing statement that the information is true.

What happens when you use it

  • A form is created as a draft, titled with the template name, and the editor opens on it. Nothing goes live until you publish.
  • Section dividers, options, required fields and the terms statement come already set up.
  • From then on the form is yours and no longer tied to the template: future template changes do not affect what you created, and your changes do not affect the template.
  • No template includes a signature. If your form needs one, add the Signature field in the editor.

What to adjust afterwards

Everything. In the Edit tab, Fields card, you change the title, questions, options, type and required setting, remove what you do not use and add what is missing. Clínica Vitta renamed the form "Facial intake — Vitta", removed the isotretinoin question (the clinic does not do peels) and added a multiple choice question "How did you hear about us?".

Which language the template is created in

The template comes in your dashboard language (Preferences › Language): Portuguese, English or Spanish. Questions, options and the Form language (Settings card) are created in that language.

In English and Spanish, templates use fields that work in any country: the identification asks for an ID document (any country), phone and address open in the respondent's country, and amounts come in US dollars (you can pick another currency per field).

Changing the Form language later only changes the buttons and messages respondents see; the questions stay as you wrote them. To get the questions in Spanish, switch your dashboard language before using the template.

Who can use it

Owner and Editor create forms from templates or from scratch. A Viewer does not see the Templates menu, because they do not create forms.

Where to go next

  • Field types (Fields collection) — what each template question validates and how to change its type.
  • ID document, address, phone and amounts (Fields collection) — ID document of any country, address and the currency of amounts.
  • Terms acceptance and proof of consent (Signature, terms and PDF collection) — the statement that closes the intake forms.
  • Draft, saving and publishing (Editing and publishing collection) — putting the form live.

In short: on Templates you create a blank form or pick one of the 60 templates by group, check the preview and use it; the form is created as a draft, in your dashboard language, independent from the template and fully editable.
