Response PDF

Each response can become a PDF ready to print, file or send: the questions and answers laid out and, when present, the signature and the pages that prove signature and consent. With the Response PDF card, the document carries your business's look. At Clínica Vitta, in Miami, each completed medical history form becomes a PDF that goes into the patient's record.

Downloading a response PDF

In Forms › (form) › Responses, click the response and then Download PDF. The file name carries the response number and the form title. Everyone who can see responses can download it, including the Viewer.

The standard document includes:

  • at the top, the form title, the account name, the response number and the received date;
  • the questions in the order of the version the person answered, with sections, text blocks and the full text of each term;
  • No answer on questions left blank;
  • the signature image, with "Signed by", name, date and time;
  • the Electronic signature record and Consent record pages, when the response has a signature or a terms acceptance;
  • in the footer, "Generated by WiseData Forms on" and the date.

Two things never go in: a question a condition hid (it was never asked, and a "No answer" on paper would look like an omission by the respondent) and hidden fields that come from the link.

Your own layout

In Forms › (form) › Edit › "Response PDF" card, turn on Use my own layout. From then on the PDF comes out without "Generated by WiseData Forms": the footer shows only "Generated on" and the date. What you can set:

  • Show the logo at the top — uses the logo the form already has, chosen in the Share tab. If the form has no logo, the card tells you.
  • Header — up to 4 lines below the logo, such as company name, tax ID and address.
  • Footer — up to 3 lines, such as phone, website or a notice.
  • Hide unanswered questions — a blank question does not appear; a section left empty disappears with it.
  • Questions that appear in the PDF — uncheck the ones that should not be in the document.

Header and footer are text only, up to 120 characters per line; blank lines are dropped. Click Save PDF layout.

Clínica Vitta set it up like this:

  • Header: "Clínica Vitta Aesthetics LLC" and "100 Example St, Miami, FL 00000".
  • Footer: "(000) 000-0000 — contact@example.com".
  • Unchecked "How did you hear about us?", which matters to marketing, not to the patient record.
  • Turned on Hide unanswered questions, so the form of a patient with no allergies does not come out with a section full of "No answer".

Details that prevent surprises

  • It takes effect right away, without publishing, from the next PDF downloaded — including for older responses.
  • Signature and terms acceptance never come out. These fields appear checked and locked with "Always appears: it is part of the document's proof.", and the record pages come out with any setting. A document missing the terms but looking complete would be a risk for whoever presents it.
  • New questions go in on their own. The card stores what you hid, not what appears: a field added later shows up in the PDF without anyone checking it.
  • Turning it off does not erase the settings. With Use my own layout off, the PDF goes back to the standard and what you set up is kept for when you turn it on again.
  • What the layout does not do: reorder questions, change colors or fonts, number pages.
  • Duplicating the form takes the layout along.

Owner and Editor set up the layout; the Viewer sees the card disabled.

Where to go next

  • Electronic signature (Signature, terms and PDF collection) — what the signature record page proves.
  • Terms acceptance and proof of consent (Signature, terms and PDF collection) — what the consent record page proves.
  • Steps, theme and logo (Appearance and after submission collection) — choosing the form's logo, which is the same one used in the PDF.
  • Exporting responses to CSV or XLSX (Responses collection) — all responses at once, as a spreadsheet.

In short: the Download PDF button on a response generates the document with the questions from the version answered, the signature and the record pages; the Response PDF card adds a logo, a text header and footer, removes the product branding, hides chosen or blank questions and takes effect right away, but never removes the signature, the terms acceptance or the records.
