Version history

Every time someone clicks Publish, the form as it is at that moment is stored as a numbered version. The history tells you what was live on each date and lets you go back when a change did not work out. At Clínica Vitta, in Miami, the medical history form is already on version 5.

Opening the history

In Forms › (form) › Edit, click Version history, at the top or at the bottom of the page. The list shows, newest first, 10 per page:

  • Version — the number, with the Live badge on the one people opening the link see.
  • Published at — date and time of publication.
  • By — who published. Publications older than this record show "—".
  • Fields — how many fields the version had. The list does not show the content; to see it, you restore it.

A form that was never published shows No versions yet.

Restoring a version

In version 5, Paula reorganized the form and removed the "Allergy history" section. Two days later, the team realized they needed it. To go back:

  1. In the history, click Restore on the Version 4 row.
  2. Confirm with Restore into the draft.
  3. Check the draft and click Publish.

What happens at each step:

  • Restoring only changes the draft. The public link stays on the live version until someone publishes. You can restore, review calmly and only then publish.
  • The current draft is replaced with the questions, logic and settings of the chosen version. If there was an unsaved change, the confirmation says so, and it is replaced too. The form title stays the current one.
  • Cards that take effect right away do not change — Accepting responses, Response alerts, Tracking and partial responses, Response PDF and Response retention stay as they are.
  • Publishing creates a new version. Restoring 4 and publishing produces Version 6, with the content of 4. Version 5 stays in the history; nothing is deleted.

If the form was changed in another tab or by someone else while the history was open, the restore is refused like any conflicting save: reload and try again.

Versions and responses

A version is never deleted or changed, because each response stays tied to the version that was live when it was submitted. Responses received on version 5 stay tied to version 5 even after 6 goes live: the PDF of each one shows the questions and the terms text exactly as the person saw them.

Who can do it

Owner and Editor open the history and restore. The Viewer does not see the button.

Where to go next

  • Draft, saving and publishing (Editing and publishing collection) — what takes effect on publishing and what takes effect right away.
  • Response PDF (Signature, terms and PDF collection) — the document for each response, in the version it was given in.

In short: each publication becomes a numbered version, with date, author and number of fields; restoring copies the chosen version into the draft without changing the link, publishing creates a new version, and no version is ever deleted, because responses stay tied to the version they were submitted in.
